    Social isolation is a growing issue, severely affecting ageing populations and is both physically and psychologically detrimental, putting a strain on the healthcare system.
    Interviewing older people and professionals in the area was key in generating and developing our concept, ‘Grace’.
    An AI-powered dynamic service database, supporting Social Prescribing by providing Link Workers an always-updated list of local services that they can refer socially isolated older people to.
    The information is validated through AI-driven phone calls, saving hundreds of hours of calling time for Link Workers.
